That means you both hit each other at the same time during the battle.

My bet would be on a poisoned arrow, meeting its purpose after a while

But Majestic is right, it happened before in other battles as well.

Gospodar Vosdovca, it's not unusual knights in battle / duels to hit each other almost in the same time.
The system cannot say: They hit each other, Lady Khia made a nasty cut to the Knight Vosdovica, but Gospodar was stronger, with a powerful blow take her out first.
Also the way system write down the blows done in (almost) the same time is random, in your case it happen that system announce yours first.
Please don't be upset that a girl took you out, you'll be surprised how good are the girls with a sword, it happen to me more than once and every time I took the opportunity to compliment the lady and come closer, if you catch my drift
